What is a Community Health Needs Assessment (CHNA)?
A CHNA is a comprehensive examination of the health status of a community. The CHNA is used to help partners in the local public health system identify areas of concern and to guide community health improvement planning efforts. 

How often is this done?
Live Well Mercer County is our local group of collaborative partners who work together to conduct the CHNA every three years. After each CHNA is complete, the partners develop a three-year Community Health Improvement Plan (CHIP). The current cycle has been made longer due to the COVID-19 pandemic. The most recently completed CHNA and CHIP are found in the 2022 Mercer County Community Health Report

How can Mercer County Residents Help?
All Mercer County residents are encouraged to complete the 2024 CHNA Survey before April 30! All information collected is anonymous. The survey can be completed online using the link above or can be printed, completed, and submitted to the Mercer County Health District using one of the versions below.
